Pharmacological Treatment of Primary Chronic Osteomyelitis: A Case Report

INTRODUCTION: Chronic primary osteomyelitis (CPO) is a rare disease, defined as chronic inflammatory process involving cortical and medullary bone. In the maxillofacial region, it mainly affects mandible, with predilection for females in wide age range, unknown etiology or treatment protocol. OBJECTIVE: The objective of this article to report case disease Primary Osteomyelitis, focusing on its clinical, radiographic, histopathological characteristics, discuss nomenclature differential diagnosis while comparing other osteomyelitis. CASE REPORT: MLP patient, 53 years old, female, leukoderma, was admitted at Maxillofacial Surgery Traumatology Service University Hospital USP referred from another service reported severe pain swelling face approximately 1 .5 year. On clinical examination, he presented edema 1+/4+ lower third right hemiface, sensitive palpation, trismus, lip paresthesia, no evidence infectious odontogenic focus, increased volume intraoral secretion. patient submitted an incisional biopsy, computed tomography scintigraphy, which showed sclerotic pattern uptake respectively. After months antibiotic therapy results, initially 20mg Prednisone regression up 5mg two doses 60mg Pamidronate resulted remission pain, trismus. Until publication article, had recurrence symptoms, totalizing 6 follow up. CONCLUSION: CPO challenging disease. effective trismus decreasement control, however more studies are necessary determine definitive condition.
